ONSITE MEDICAL AND BEHAVIORAL HEALTH PHYSICIANS AND SERVICES

Hopess believes access to care in early recovery is necessary and essential. Therefore, Hopess has onsite and/or telemed physicians that can be accessed 24 hours a day. Once our residents establish care with our providers, they can continue seeing their providers after their services end with us. Our physicians can assist with all health care needs. Some of these include:

  • Immediate access to care
  • Refilling prescriptions
  • Psychological Evaluations
  • Psychiatric Treatment
  • Genetic testing to determine medication effectiveness and what medications will help the most
  • Physicals and Health Screenings
  • Effective management of routine, chronic and specialty issues
  • TB Tests
  • Onsite Lab work
  • Onsite imaging services
  • Urine Drug Screens
  • Pregnancy Tests and
  • Covid Testing
  • Preventive Services, vaccinations, and care for long-term conditions
  • Disease Education and Management
  • Coordination of Care with specialty providers
  • Establishing a bank account (even when turned down due to owing other banks fees)

CASE MANAGEMENT SERVICES

Hopess offers one-of-a-kind treatment that includes onsite case managers. Our case managers are an integral part of your treatment. They will help you rebuild the life you want. They can assist you with any of your individual needs. Some of these include:

  • Obtaining ID/Social Security Card/Birth Certificate
  • Obtaining food stamps
  • Disability paperwork
  • Obtaining an SMI Evaluation
  • Support for court appointments
  • Job placement/Resume writing/Clothing for interviews
  • Peer Support Classes
  • Dental Needs
  • Medical Needs
  • Ancillary Behavioral Health needs
  • Eyeglasses/Contacts
  • Housing
  • Enrolling in School
  • Obtaining GED or High School Diploma
  • Engagement in Cultural Activities

INTERNSHIPS

Hopess has created internships that allow residents to learn needed skills in the following areas:

  • Food Service (including obtaining food handlers license)
  • Cleaning and Janitorial Services
  • Peer Support
  • Handyman Services
  • Plumbing Helper
  • Painting Helper
  • Receptionist/Administrative Assistant

AFTERCARE

Hopess has partnered with Carelock LLC – a full continuum Integrated Health Clinic to provide it’s medical and behavioral health outpatient services. Through their Lighthouse Program, our members can continue their treatment by attending and engaging in the following services with the Hopess staff that they have connected with. Client’s may engage in some or all services. For example, if a client discharges from residential services, our case management or medical/behavioral health team that followed them during their residential stay can still follow them if they go to another program in Arizona. The most critical time in a person’s recovery is not when they are enrolled in residential services, but rather when they discharge. We want to make sure that our residents continue to receive support, services, and engagement after they leave us. We can wrap a person as tightly as needed after they discharge to ensure their success. Some of these services include:
